[XXX], [XXX] PM. 2024 Toyota Tundra Hybrid (i-FORCE MAX, V35A engine), 36k miles, original owner. Clear, dry pavement, evening daylight. Exiting [XXX] westbound at [XXX] , I merged into the far right lane at the second light for a right turn. A vehicle was stopped ahead at the light. Traveling 15-20 mph, I applied the brake for controlled deceleration. Right foot squarely on brake, not touching accelerator. Vehicle did not decelerate as expected. For 3-4 seconds the brake felt unresponsive — pedal felt soft and loose, lacking normal firmness. I pressed the brake fully to the floor. A dashboard warning appeared: “gas and brake pedals are being pressed simultaneously.” I was not, and had not been, touching the accelerator. I then heard engine RPM increase audibly, consistent with an acceleration command, while my foot remained fully on the brake. I felt forward thrust working against my braking. The warning cleared and brakes engaged. Vehicle stopped approximately half a car length from the vehicle ahead, narrowly avoiding rear-end collision. Automatic Emergency Braking did NOT engage despite conditions clearly meeting its criteria — stationary vehicle directly ahead, my vehicle approaching at 15-20 mph with no observable deceleration for 3-4 seconds. No collision warning sounded; no auto-braking intervened. Notably, this same AEB has previously engaged falsely in benign situations (e.g., backing near low vegetation), suggesting reliability/calibration issues. Prior manufacturer notice: On [XXX] at 15,412 miles, I reported to Camelback Toyota, Phoenix (RO #[XXX], on file): “vehicle lurches forward going from reverse to drive (now more consistent)” — dispositioned by tech 10210 as “vehicle is operating to spec,” no diagnosis performed. Same RO documented “rattling in neutral/at stop” — dispositioned “unable to duplicate.” Toyota was on notice of throttle-response symptoms 21 months before this safety event. Vehicle Information: 2024 Toyota Tundra 4x4, VIN: [XXX] livery: [XXX] Current Mileage: 24,147 Complaint Synopsis: My 2024 Toyota Tundra is subject to two active open safety recalls with no remedy available ("NO FIX YET" status per Toyota owner portal). 1. What component(s) or system failed or malfunctioned, and is it available for inspection upon request? Primary: Engine assembly (Recall 25TB14 / 25TA14) – machining debris left in the engine during manufacturing. This can cause engine knocking, rough running, no-start, and sudden loss of motive power. Secondary: Reverse lamp system (Recall 25TB06 / 25TA06) – moisture intrusion causing both reverse lamps to become inoperable. The vehicle remains available for immediate inspection at Clearwater Toyota or by Toyota representatives. 2. How was your safety or the safety of others put at risk? The engine defect creates a severe risk of sudden loss of motive power while driving at highway speeds, which can cause loss of vehicle control and increase the likelihood of a crash. The reverse lamp defect eliminates rear visibility and brake indication when backing up in low-light conditions, significantly increasing the risk of collisions with other vehicles or pedestrians. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? The defects are formally admitted by Toyota through active NHTSA safety recalls. Clearwater Toyota has refused multiple requests for inspection and service documentation. No independent inspection has been performed due to Toyota’s refusal to schedule one. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No. Toyota has not conducted a physical inspection of my vehicle despite repeated requests. Clearwater Toyota provided a temporary loaner vehicle but has refused to inspect or document the recalled conditions. TOYOTA TUNDRA 2024 SERVICE BRAKES

Parked on incline. Vehicle off. Vehicle automatically locked doors without operator intervention. Returned to vehicle. Vehicle beeped with associated lights flash when operator touched driver's extenal door handle. IMMEDIATELY the vehicle began rolling backwards downhill towards oncoming traffic. I was able to hop on running board, jump into driver's seat, press brake and start vehicle. The brakes engaged and the vehicle stopped before hitting oncoming traffic. Dealer found CODE P15AD87. Battery State Sensor (#28850-F4010) was replaced. Vehicle currently has 2,000 miles since repair on 01/23/26 without reoccurrence.

I was stopped and went to pull out of the gas station. With my foot on the throttle I pull out. And it would not accelerate. I had to pump the gas pedal I almost got hit by oncoming traffic due to the lack of acceleration this has happened multiple times Very dangerous
