Incident Report – 2026 Tesla Model Y Date of Incident:XXX Time of Incident: 12:30 am Location: [XXX] I was operating my 2026 Tesla Model Y and was driving manually. Full Self-Driving (FSD), Autosteer, and other self-driving functions were not engaged at the time of the incident. While traveling on Highway 16 at approximately 60 mph, the vehicle approached the right-side white lane line. At that point, I experienced a sudden steering intervention that turned the vehicle sharply to the left. The steering input was unexpected and occurred while I was manually controlling the vehicle. Following the steering intervention, the vehicle crossed the centerline into the opposing lane of travel and continued off the roadway into the ditch on the opposite side of the road. The incident resulted in damage to the vehicle. Road conditions were dry. Weather conditions were safe. Lane markings were clear. I am requesting that Tesla preserve and review all vehicle telemetry, event logs, steering-assistance records, and any other relevant data associated with this incident. Specifically, I request an investigation into whether Lane Departure Avoidance, Emergency Lane Departure Avoidance, or any other driver-assistance feature activated and contributed to the steering maneuver. I am also requesting a determination regarding responsibility for the resulting vehicle damage and whether Tesla will reimburse repair costs if a system malfunction or defect is identified. Supporting documentation, including photographs, dashcam footage (if available), repair estimates, and other evidence, can be provided upon request. The contact owns a 2026 Tesla Model Y. The contact stated that while driving at 65 MPH, with the autopilot feature engaged, the vehicle approached a lane closure as big, metal arrow road signs where on the road indicating the road closure. The contact, who was distracted at the time, expected the vehicle to acknowledge the road closure and switch lanes. The vehicle failed to recognize the Chevron barricade lights on the highway and the vehicle ran through the road signs without warning. The contact was forced to take over and stop the vehicle due to the failure. Due to the failure, the front, driver side mirror detached from the vehicle. The contact exited the highway and inspected the damage to the vehicle. Despite the failure, the contact managed to drive the vehicle home. The service center was notified of the failure and an appointment was made to have the damages repaired. The cause of the failure had yet to be determined. The vehicle was not repaired. The failure mileage was approximately 21,000.

The car is unstable on the highway when there is a crosswind - either from wind gusts or from large vehicles. The car is easily pushed by wind until lane assist activates to stop it from going outside of the lane. It feels very unsafe to drive on the highway. This is how the car was when it was bought. An alignment was done during the first Tesla service visit and that helped slightly. The second Tesla service visit they said the vehicle was operating as expected and then checked wheel balance after I insisted. They said they fixed wheel balance but it still did not change how the car handles in crosswind. The third Tesla service visit (different service center) they said the vehicle was working as expected and said they could not drive for a significant distance to test. If lane assist ever fails when I'm not ready to instantly react to a gust of wind, it feels like it would easily move into another lane or off the road. When passing large vehicles where wind changes direction, the car sometimes gets pushed left and right which sometimes triggers an alarm and shows an on screen message saying to take control of steering. I test drove another of the same model car for an hour and had the same unstable feeling in the wind but was unable to test long enough to find an area with high crosswind.

Second time in the five months I have owned the car that I need to replace the windshield in its entirety. Not only does it chip incredibly easily, but any chip immediately becomes a large crack, usually overnight and definitely before Safelite or a similar company can be called to try to fix it. In addition to being dangerously fragile and very expensive to fix - although very lucrative for Tesla since I can’t drive without an intact windshield - it’s very difficult to get an appointment. This time around I must drive around with an ever-expanding crack for 16 days before driving 100+ miles to the nearest Tesla service center to get it replaced. How can it possibly keep me and my family safe if it breaks that easily and that often and why is it normal to replace the windshield that often in any car much less a brand new car?
